How to dispute the accuracy of information on your background report

If you have already obtained a copy of your background report and wish to dispute the accuracy of any of the information contained within the report, you can do so by either Fax, Mail or Phone. If you do not have a copy of your background report, please click here.

Fax in a dispute

  • Fax your dispute to the Consumer Disclosure Department at 1-888-495-8476.
  • Your dispute should include:
    • Your full name
    • Your date of birth
    • Your address
    • Your phone number
    • Your full Social Security Number (for identification purposes)
  • The details of your dispute and any relevant documentation
